Outcomes

The LINk has developed some outcomes that we’re working to deliver. An Outcome is the difference that has been made when one or more activities have taken place. They are the things we want to see achieved in Tameside over the next few years.

The Outcomes we have developed are:

  1. Tameside has health and adult social care services that are shaped to meet people’s needs and are improved as a result of people’s experiences.
  2. LINk members feel their voice is being heard and have confidence in the validity and transparency of decision making in health and social care.
  3. Local people have been in contact the LINk and are satisfied with the service they received.
  4. LINk members are aware of local health and adult social care issues, they know how to access services and know the appropriate route to resolve their problems.
  5. The LINk has constructive and open relationships with health and adult social care commissioners, managers and providers.
  6. The LINk membership is representative of the population of Tameside, including harder to reach and seldom heard groups. All sections of the community are provided with suitable avenues and opportunities to give their views and get involved.
  7. The LINk’s host supports and maintains the LINk within budget.
The LINk cannot deliver all of these outcomes on its own and doesn’t want to replace the excellent involvement and engagement work going on in our partner organisations. One of our guiding principles is that we seek to collaborate with and add to the work of our partners so that we can deliver these outcomes together.
 
Projects
To help to meet these outcomes projects have been chosen for 2009/10. These were chosen based on the results of the large survey we did early in 2009 and the workshops we held at our Annual Celebration Event in July 2009.

The main work streams are:

  • Personalisation of Adult Social Care (work starting September 2009)
  • Community Based Mental Health Support (work starting November 2009)
  • Access to GP (Family Doctor) Services (work starting January 2010)
  • Access to NHS Dentists (work starting March 2010)
  • Care at Tameside Hospital
  • Care Homes project
